Privacy Policy

I. General Provisions

The controller of personal data pursuant to Article 4(7) of Regulation (EU) 2016/679 of the European Parliament and of the Council on the protection of natural persons with regard to the processing of personal data and on the free movement of such data (hereinafter: "GDPR") is MILENIUM DENT s.r.o., Business ID: 08852073, with registered office at Budečská 2165/35, Vinohrady (Prague 2), 120 00 Prague (hereinafter: "controller").

Personal data means any information relating to an identified or identifiable natural person; an identifiable natural person is one who can be identified, directly or indirectly, in particular by reference to an identifier such as a name, identification number, location data, online identifier or to one or more factors specific to the physical, physiological, genetic, mental, economic, cultural or social identity of that natural person.
The controller has not appointed a data protection officer.

II. Sources and Categories of Personal Data Processed

The controller processes personal data that you have provided through the contact form on the website:

  • first and last name
  • phone number
  • email address
  • optional note
  • approximate date you wish to book an appointment

The controller only processes your identification and contact data for the purposes stated in Article III.

III. Legal Basis and Purpose of Personal Data Processing

The legal basis for processing personal data is your consent to the processing of personal data for the purpose of handling your inquiry from the contact form pursuant to Article 6(1)(a) GDPR. The purpose of processing personal data is to handle your inquiry from the contact form and to exercise rights and obligations related thereto. The controller does not carry out automated individual decision-making within the meaning of Article 22 GDPR.

IV. Data Retention Period

The controller retains personal data for the period necessary to handle your inquiry and to assert claims arising from such inquiries (for a period of 2 years from the handling of the inquiry). After the retention period expires, the controller will delete your personal data.

V. Recipients of Personal Data (Controller's Subprocessors)

Recipients of personal data are persons involved in handling your inquiry. The controller does not intend to transfer personal data to a third country (outside the EU) or to an international organization.

VI. Your Rights

  • the right to rectification of personal data pursuant to Article 16 GDPR, or restriction of processing pursuant to Article 18 GDPR
  • the right to erasure of personal data pursuant to Article 17 GDPR

  • the right to data portability pursuant to Article 20 GDPR
  • the right to withdraw consent to processing in writing or electronically at the address or email of the controller specified in Article I of these terms

You also have the right to lodge a complaint with the Office for Personal Data Protection if you believe that your right to personal data protection has been violated.

VII. Personal Data Security Measures

The controller declares that it has taken all appropriate technical and organizational measures to secure personal data. The controller has implemented technical measures to secure data repositories and repositories of personal data in paper form, including password-protected access and antivirus protection. The controller declares that only authorized persons have access to personal data.

VIII. Final Provisions

By submitting the contact form from the website, you confirm that you are familiar with the terms of personal data protection and that you accept them in their entirety. You consent to these terms by submitting the contact form, which is explicitly and clearly marked to indicate that by submitting it you agree to these terms. The controller is entitled to amend these terms. A new version of the privacy policy will be published on its website and simultaneously sent to your email address that you have provided to the controller.

These terms come into effect on August 1, 2024.
